Brand new in 2009, the elegant 124-bed facility offers a wide range of care including skilled nursing, rehabilitation, long-term care, respite care and restorative nursing care. Our dedicated staff and therapy team excel in their professional fields, so our residents receive the five-star care and attention that they so richly deserve.
By incorporating our state-of-the-art therapy gym with specialized physical, occupational, and speech therapy programs, our objective is to give our residents expert, individualized care to help them achieve the highest level of recovery and independence possible. Most of our short-term residents are rehabilitated and able to return home within 30 days of admission.
For those who stay with us on a long-term basis, we are committed to enhancing their lives every day by providing professional and personalized care with social activities seven days a week. We enjoy shopping trips, Bingo, meditation, movies, singing and much more!

Admission to Senior Care Health & Rehabilitation Center Decatur is initiated by a Physician, a Social Worker/Discharge Planner or a family member. We offer referral response within two hours and we accept admissions seven days a week.
For financial peace of mind, we accept Medicare, Medicaid, LTC insurance, private pay or Medicare Replacement Insurance. We will happily help educate our families in the application process for Medicaid benefits.
